This examination involves a written test for individuals who wish to become active as a Medication Aide on the Nebraska Medication Aide Registry.  The registration fee includes admission to one Medication Aide examination.  Pencils, test booklets and answer sheets will be provided.
 
***Please note: Bring an original photo ID or Driver's License for identification.  No copies will be accepted.  Once the test has begun no one will be admitted, so please be sure to be on time.***

To register with a money order payment: A printable registration form is available at the bottom of the website. Please print, fill out, and mail the form along with your money order payment. No personal checks, please.
 
Cancellation Policy: Cancellations may be rescheduled for a future testing date/time with PHCI.
